How do I access my child’s medical information?

Your child’s patient records, lab results, immunizations and other medical information from appointments in our office after 10/3/22 are accessible through the My Health Connection (MHC) patient portal account.  If you do not have an account, please use the link below to sign up.

Any appointments that occured before 10/3/22, the medical information is accessible through our previous patient portal Follow My Health (FMH).  There currently is not an end of life planned for the “Follow My Health” portal. State laws require the Youth Clinic to maintain medical records for a period of ten (10) years.

How do I pay my bill online?

Does the My Health Connection (MHC) patient portal account have my child's past medical records (prior to 10/3/2022)?

No, however you can still access your child’s previous medical records through the Follow My Health (FMH) patient portal. Additionally, you can contact our Medical Records department to obtain complete medical records.

When did The Youth Clinic go live on the community UCHealth EPIC Electronic Health Record?

Our practice went live on UCHealth EPIC on October 3rd, 2022.

Why does the My Health Connection (MHC) patient portal account have UCHealth on it?

UCHealth developed the My Health Connection (MHC) patient portal in the effort to offer community patients and providers access to a complete medical record including; hospital stays, primary care visits and community specialist records.

Are The Youth Clinic and Pediatric Urgent Care owned by UCHealth?

No, the Youth Clinic and Pediatric Urgent Care have an agreement with UCHealth to access their community electronic health record (EHR). This partnership already exists with many community providers enabling continuity of care across our medical community.

What has happened to my child’s medical records now that The Youth Clinic is on UCHealth EPIC electronic health record (EHR)?

Your child’s medical records prior to  10/3/2022 (when The Youth Clinic joined the UCHealth community EHR) are accessible through our previous patient portal Follow My Health (FMH).  You are also always able to request copies of your child’s medical records from our office.

Medical records from your child’s visits on and after 10/3/22 , are accessible through the My Health Connection (MHC) patient portal account.

How long will I be able to access records from Follow My Health (FMH)?

I already have a My Health Connection (MHC) patient portal account, how do I sign up my child for a portal account?

If your child is 14 or under you must first add yourself as a My Health Connection (MHC) patient. Once you have your own portal account you can sign up to get access to your child’s account.

 

My child is over the age of 14, how do I get access to their My Health Connection (MHC) patient portal account?

Can I message my child's provider through My Health Connection (MHC) patient portal account?

Yes.  To message your child’s provider a My Health Connection patient portal account is required.  It is also important you select your child’s Youth Clinic provider as the Primary Care Provider (PCP).

Why did The Youth Clinic switch to the community UCHealth EPIC electronic health record?

It is of highest importance for our providers to have access to a patient’s complete medical record including records from specialists, hospitals, and other community providers, to ensure your child is receiving appropriate and coordinated care.
