Part-Time Clinical Nurse Liaison Registered Nurse (RN) or Licensed Practical Nurse(LPN)

Fort Collins Youth Clinic is hiring a part-time (28 Hours/Week) R.N. or L.P.N. to act as a primary communication bridge between patients and healthcare providers by assessing, triaging and/or resolving patient incoming requests.

Pay Range:

RN – $28.50 – $31.00 per hour

LPN – $21.75 – $24.73 per hour

MAJOR RESPONSIBILITIES

Clinical Practice and Quality of Care:

  • Complete medication refill requests in a timely manner following established clinic guidelines

  • Monitor incoming patient messages and triage requests in a timely manner following established clinic guidelines

  • Complete accurate and timely documentation of patient communications in the electronic medical record

  • Schedule appointments following established clinic guidelines

  • Follow up with patient/parent as needed per provider requests

  • Cross trains as phone triage nurse

In addition but not limited to the following:

  • Telephone and in-person triage (for symptom management)
  • Review and act on test results (lab, x-ray)
  • Review and manage communications from internal and external “customers”
  • Initiate and monitor intravenous (IV) infusions
  • Assist or perform procedures (e.g. ECGs, dressing changes, subcutaneous or intra-muscular injections, etc.)
  • Provide patient education
  • Execute delegation protocols
  • Assist with medication management (e.g. medication renewals)
  • Complete accurate and timely documentation in the electronic medical record
  • Assist with management of urgent and emergent patient care situations
  • Serve as a mentor and resource to others
  • Respond to medical emergencies
  • Assist with education of nursing students
  • Work independently within their scope of practice to achieve clinical outcomes
  • Maintain a level of professional development through continuing education, quality improvement initiatives and sharing of knowledge

Skills/Qualifications

  • Graduate of accredited School of Nursing, holding current Colorado Registered Nurse or Licensed Practical Nurse license

  • BLS certified

  • Great customer service skills – Ability to establish and maintain effective, pleasant working relationships with patients, employees, physicians and the public

  • Ability to understand HIPAA regulations with regard to patient confidentiality and to respect patient confidentiality and privacy by maintaining the security of all patient communications. 

  • Excellent organizational skills and attention to detail

  • Ability to work independently and multi-task in a fast-paced office environment
