If these steps are not followed the record of your vaccine will not be pulled into your medical chart:
Step 1
Enter your contact info to search for an existing Primary Health account using your phone # or email.

- Look for email to confirm your profile
- Choose a profile and follow instructions to complete appt scheduling
– Go to Step 2 if your profile was not found
– Skip Step 2 if your profile was found
Step 2
If invalid info was provided or no account was found, click Search button again (No need to contact Primary support).
- Click “Register Here” button
- Enter “The Youth Clinic” to search for site
- Find The Youth Clinic locations and click Continue
- Click “Default Registration”
Proceed with instructions and register patient information using full legal name.
